Phase One

Selecting Situation Cards

Ask the client to select four to six situation cards which represents important communication in their daily life.

The client can either select the cards in the waiting room or at the beginning of the appointment. An instruction card is provided in the pack. 

Phase Two

Being Curious

Once the client has selected the situation cards, ask open-ended questions and listen actively to their response.

For example, you might say, "Tell me about the situation. What is important about it and how do you manage?"

Phase Three

Rate Ease and Importance

Ask the client to place the selected situation cards on the rating scale according to how easy or difficult each situation is to manage.

Then ask them to use the enclosed importance markers to rank the situations.

Phase Four

Identify and Explore Strategies

With an understanding of the client's challenges and priorities, you can explore which effective communication strategies they are already using and identify new ways to use these strategies in more challenging situations.

Two sets of strategy cards are provided for you and the client to place with each of the situations selected. One set of strategy cards is for communication strategies and the other is for technological strategies. Empty strategy cards are also provided for you to write down additional strategies.

Phase Five

Document Decisions and Strategies

You can use the documentation form provided to record which situation cards were selected, how the client rated them in terms of ease and importance, and which existing and new strategies were agreed upon for later reference.

A listening guide is provided for you to use as you meet with clients to help you listen actively to what they are saying.

Living Well Quick Tool

One-Sheet Simple Version

The Living Well Quick Tool is a great way to start a conversation with a client and learn more about communication situations that are important to them.

Since the Quick Tool version of Living Well is all contained on one sheet, it is easy to print out and give to clients in the waiting room.

Alternatively, you can send it to the client at home via email before their consultation. They can fill out the form at home, and you can discuss their answers at the appointment.

Using the Quick Tool

Here are some suggestions on how to use the Quick Tool:

  • Ask the client to look at the photos and check the box next to the communication situations they feel are relevant and important to them.

  • Talk to the client about each situation and how they manage today.

  • Discuss new strategies that can be implemented to improve their communication, set goals, and decide on a plan to achieve those goals.

  • You can use the Living Well documentation form to document the outcome of your conversation.
